Who is Eligible?
All Providence Health Plan-covered members and their qualified dependents. The participating provider will verify your eligibility when you present your Providence Health Plan member ID card.

Select a provider from the ChooseHealthy Directory or call ChooseHealthy Member Services at 1-877-335-2746.
Schedule an appointment with the participating provider. You do not need a physician referral.
Your selected provider will develop, if necessary, a treatment plan for you.¹
You may change providers at any time.
Pay for services received and/or fees directly to the provider.
There are no claim forms or additional paperwork to complete other than the provider’s usual patient history/documentation forms.
There are no annual limits to this program.
