Live Healthy Lane’s 2015 Community Health Needs Assessment (CHNA) is a collaborative project between United Way of Lane County, Lane County Public Health, PeaceHealth Oregon West, and Trillium CCO. Improving the health and well-being of Lane County is core work of the partnering organizations. The overall goal of this process is to capture a full picture of our community’s health and identify key priority areas where the community (including you!) can then take action.
