Family Support Navigator is responsible for creating a customized care plan and for guiding youth (ages 0-18) and their families through the needed services. This person will coordinate the child and family teams (e.g., family and drug/alcohol therapist), and community members to provide needed services. The Navigator will organize services at various locations in Clackamas County as well as work collaboratively with other service providers and program partners to coordinate an ongoing caseload of participants to provide services including parent engagement. Work cooperatively with the Clackamas County Lead Navigator and the interagency collaboration.

This position requires frequent and lengthy standing, speaking in a loud voice and ability to lift at least 30 pounds. Also, work is often performed in an office environment and involves frequent use of a computer. Frequent use of phones to communicate with current and potential partners and volunteers. Will drive to perform work with the community. May require occasional travel out-of-town. Availability to work evenings is required.

Salary: $2,916 – $3,100 per month.
